What is DVA Community Nursing?

The DVA Community Nursing Program provides essential in-home nursing care to support veterans and war widows with their health and personal care needs. This program ensures that individuals can maintain their independence and wellbeing while receiving expert medical attention

At Holicare, we will design nursing services to meet the specific needs, values and aspirations of you and your family.

Our DVA Community Nursing Services Include:

  • Clinical Nursing – Medication management, wound care, and ongoing health monitoring.

  • Personal Care – Assistance with daily tasks such as bathing, dressing and hygiene.

  • Chronic Disease Support – Helping you manage long-term conditions with professional care.

  • Post-Hospital Recovery Care – Support with rehabilitation and ongoing treatment at home.

  • Carer Support Services – Helping family carers by providing professional nursing assistance.

  • Continence Management – Compassionate care to assist with bladder and bowel health for improved comfort and dignity.

Are You Eligible?

If you are a veteran or war widow/widower with a DVA Gold or White Card, you may qualify for government funded community nursing services. You mut also undertake an assessment to determine your need for clinical and/or personal care.

To receive government funded nursing care at home, you must obtain an authorised referral from:

  • A General Practitioner (GP)
  • A Nurse Practitioner specialising in community nursing
  • A Treating Doctor in a Hospital
  • A Hospital Discharge Planner

Once you have obtained a referral, our DVA Care Coordinator will contact you to:

  • Arrange a free consultation at a convenient time
  • Guide you through the community nursing assessment process
  • Connect you with our experienced Registered Nurses for personalised home care

If you're unsure about eligibility, we can assist you in navigating the process.

What are the costs?

There is no cost to you if you are assessed as eligible for assistance from the DVA Community Nursing Program. For more information, contact our office on (03) 8374 7603 or visit the Department of Veteran Affairs website.
