Case Manager - HCP
About the Role:
We have an exciting opportunity to join Holicare as a Case Manager for Home Care Package servicing Victoria Metro area. This is a full-time permanent role!
The role will consist of promoting and attracting new referrers and customers to Holicare’s HCP program Level 1-4, attend and present at meetings with stakeholders to grow client base and private clients. This role is predominantly based in the field, working closely with clients, and attend office-based meetings to liaise with other team members of the Case management team. Travel is required.
As a Case Manager, you will:
- • Promote Holicare HCP program in the community and potential clients.
- • Attend / present in client meetings and conduct assessment, referral, review and goal planning
- • Manage client funds management and developing budgets for clients
- • Relationship management with internal and external stakeholders
- • Co-ordinate and manage services to our Home Care Packaged clients
- • Assist in business development opportunities
Essentials:
- • Demonstrated experience (2+ years) in Case Management in the Health, Aged Care, Disability or Community Services Sectors, including • government-funded care
- • Cert III, Cert IV in Community Services, or higher
- • Current Police Certificate, First Aid
- • Current valid Australian Driver’s license
- • Proficient in MS Office suite of applications
- • Strong organisational skills with ability to multitask in busy environment

Community Registered Nurses
About the Role:

As a Community Registered Nurse, you will:

- • Provide a range of services including but not limited to, wound care, chronic disease management, medication management, • continence management, arranging carer support, and personal care
- • Complete all tasks and documentation to a high-quality standard
- • Educate clients and their families, providing a multi-disciplinary approach
- • Collaborate with health professionals and other stakeholders to achieve client outcomes
Essentials:
- • Unrestricted current registration with APHRA
- • Sound clinical and assessment skills
- • Experience within a community setting

As a Personal Care Worker, you will:
- • Be compassionate, reliable and respectful
- • Assist with all areas of personal care including bathing/showering, hygiene, dressing, grooming, fitting of aids and eating meals
- • Assist with domestic duties and other activities of daily living
- • Comply with operational guidelines, and task directives
- • Work collaboratively as part of a team
- • Build rapport with families, team members and other stakeholders
Essentials:
- • Three (3) years’ experience as a Personal Care Worker
- • Certificate III in Home and Community Care, Aged Care or Disability, or Certificate III in Individual support
- • Well-developed communication and interpersonal skills
- • Reasonable degree of physical fitness
- • Flexibility to work available shifts at short notice
- • Your own reliable vehicle
- • Current Police Clearance certificate
- • Working with Vulnerable/ Aged care people specific check

- • Manual Handling competency
